What should I expect from Dr. Mauldin’s office when I visit the first time?

We strive to always greet our guests with smiling faces and hospitable attitudes. We know for many it can be stressful coming to the dentist, so we do our best to make you feel as comfortable as possible.

After being greeted with a smile, we will check you in for your appointment and be sure all the correct paperwork and information is provided. The patient forms help us get to know you better and provide the best plan for treatments. Get a head start by filling out your new patient forms! During your appointment, we will talk through treatment options and what works best for you.


What do I need to bring with me to my first appointment?

We only need you to bring along a few things for your first visit to our office:

1. Patient form

2. If you are planning to use insurance for payment, the name, social security number, date of birth and employer of the insured must be provided either before the first visit or the day of the first visit.
